I consent to SRIT India Limited securely processing and sharing my personal and health data with the selected Registered Medical Practitioner (RMP) for this service. I acknowledge that OPLive App is strictly a technology intermediary and that the RMP is an independent professional solely responsible for all medical advice, diagnoses, and treatment.
I understand the inherent limitations of remote care (when opting for teleconsultation), including the absence of a physical examination, the possibility of provisional diagnoses, and the requirement to seek an in-person consultation or diagnostic tests if advised by the RMP.
I voluntarily consent to a remote medical consultation via real-time audio and video with the doctor, acknowledging that unforeseen technical disruptions may occur and the provider will attempt to reconnect or call if disconnected.
I confirm that standard medical privacy laws apply and I am joining from a private, secure location; neither party is permitted to record (audio, video, or screenshot) this session without prior written consent from both sides. OPLive App does not record the audio or video of any consultation, and no consultation audio or video is stored on OPLive App's systems. Only the consultation record, prescription and consultation summary generated by the Registered Medical Practitioner are retained, in accordance with the Data Retention & Deletion Policy.
I (user) am the parent/legal guardian, providing consent and taking responsibility on behalf of the minor patient (if added) for this consultation, health data sharing, and all declarations mentioned above.
Consent is captured as a mandatory electronic acknowledgement before the consultation commences. The Platform securely records the User ID, Consultation ID, Timestamp (IST), IP Address, Device Type, Consent Form Version, and method of acknowledgement in a tamper-evident audit log. Consent records shall be retained for a minimum period of three (3) years after completion of the consultation or for such longer period as may be required under applicable law.
